Client Acknowledgment & Agreement

By creating an account and accessing the TrueBalance Veterinary Acupuncture, LLC. client portal, I acknowledge and agree to the following:

• I have read and understand the Client Policies and Procedures of TrueBalance Veterinary Acupuncture, LLC., and agree to be bound by them.
• I confirm that I am the lawful owner or authorized agent of the pet(s) listed in my account and that all information provided is true and accurate to the best of my knowledge.
• I authorize the veterinarian(s) of TrueBalance Veterinary Acupuncture, LLC. to examine, diagnose, prescribe for, and/or treat my pet(s) as medically appropriate. I understand that no medical outcomes can be guaranteed.
• I accept full financial responsibility for all services rendered and understand that payment is due at the time services are provided unless otherwise agreed upon in writing.
• I authorize TrueBalance Veterinary Acupuncture, LLC. to charge the payment method on file for services provided, including outstanding balances, late cancellations, or fees agreed to under practice policies.
• I consent to receive communications related to my pet’s care, scheduling, reminders, and billing via email, text message, and/or mobile applications.
• I confirm that I am at least 18 years of age.
• I grant permission for the use of photos and/or videos of my pet(s) and/or myself with my pet(s) for educational, marketing, and social media purposes unless I explicitly opt out in writing.
• I understand that my pet’s medical information will remain confidential and will not be released without my consent, except as required for continuity of care with another veterinary facility or as required by law.
